"以及" can only connect phrases,not words. The phrases after "以及" is commonly considered to secondary. "与" and "和" are used to express the relationship. "与" is more elegant than "和". such as "老人与海". "和" is mostly used in oral form.
2017年4月13日 · 以及 sounds formal. It is rarely used in spoken language but acceptable, okay in casual writing and common in formal writing. 还有 sounds very colloquial. It's fine in casual writing but should be avoided in formal writing. It's not true to say 以及 only connects phrases. A quick search will give you many good counter-examples.
2012年8月29日 · P.S.以及 is the most formal one, which means "additionally", usually used in writing rather than in daily conversation. You can seldom hear 以及 from a native Chinese speaker unless in a speech, report or else.
